Health-e-Profile Health Assessment
Find out how healthy you really are!
TUH cares about you and your family's health and Health-e-Profile is a tool that you can use to achieve wellness.
Health-e-Profile is an online health assessment which gives you an easy way to evaluate your health from the comfort of your own home. Just fill in information about your lifestyle, family history and health - and you will receive an instant health report with practical suggestions for steps you can take to improve your risk factors and live a longer, healthier life.
Find out what you are doing right, where you could improve, and learn simple steps you can take that will make a big difference to your health. Your health report will alert you to risks you might have, and just for fun you will also find out your “health age” and how you can grow younger!
It does not take a lot of time to complete the health assessment, but it could give you some in the long run!
Remember to:
- Allow approximately 15 minutes to compete the survey
- Have your latest health check results handy, such as blood pressure and cholesterol. The more information you can provide, the more accurate your results will be. However you can still complete the survey even if you do not know this information.
- Be honest! Your health age can only be helpful if you provide honest answers to each question.
Confidentiality is guaranteed
Your online health assessment is completely confidential as you are the only one that will be viewing your health report.

How to access Health-e-Profile
Health-e-Profile is in the secure area of our website under Member Services Online.
- The primary member will need to register for Member Services Online.
a. If you do not have an email address - during registration type in 'noemail@tuh.com.au' as your email address.
b. To register click here.
c. If you are registered, proceed to step 3. - Once you have submitted your registration you can log in to Member Services Online immediately.
- Login to Member Services Online (you will need your membership number and password).
- On the left hand menu click on Health-e-Profile.
- Select your name from the list of adult members on your policy.
- Click on the box to confirm that you are the person completing the health assessment.
- Click on the Proceed button and the Health-e-Profile questions appear.
Health-e-Profile replaces Healthtrac
Health-e-Profile replaces Healthtrac, which had been operating since 1991. TUH made the decision to replace Healthtrac to offer our members an easily accessible online health assessment and an environmentally friendly and cost-efficient approach to taking care of our members and their health.
Active Health Bonus
TUH recognises the benefits of a healthy lifestyle and the importance of providing our members with information about how to manage their own health. The Active Health Bonus is a benefit to reward members who continually participate on Health-e-Profile. Participation means that an adult member completes one health assessment in any consecutive 12 months. The Active Health Bonus benefits can be used to claim products and services that support your health.
